fix: admin token requests does not automatically have id (#6501)
To check that users do indeed have permissions to update the roles from
project-service, we've been depending on req.user.id.
We had one error on Friday March 8th, where we managed to send
undefined/null to a method that requires a number. This PR assumes that
if we have an API token, and we have admin permissions and userId is not
set we're a legacy admin token.

It uses the util method for extractUserId(req: IAuthRequest | IApiRequest), so if we've passed through the apiTokenMiddleware first, we'll have userId -42, if we haven't, we'll get -1337.

fix: setRolesForUser and setRolesForGroup role check (#6380)
In order to stop privilege escalation via
`/api/admin/projects/:project/users/:userId/roles` and
`/api/admin/projects/:project/groups/:groupId/roles` this PR adds the
same check we added to setAccess methods to the methods updating access
for these two methods.

Also adds tests that verify that we throw an exception if you try to
assign roles you do not have.

Thank you @nunogois for spotting this during testing.

feat: Adding Project access requires same role (#6270)
In order to prevent users from being able to assign roles/permissions
they don't have, this PR adds a check that the user performing the
action either is Admin, Project owner or has the same role they are
trying to grant/add.

This addAccess method is only used from Enterprise, so there will be a
separate PR there, updating how we return the roles list for a user, so
that our frontend can only present the roles a user is actually allowed
to grant.

This adds the validation to the backend to ensure that even if the
frontend thinks we're allowed to add any role to any user here, the
backend can be smart enough to stop it.

We should still update frontend as well, so that it doesn't look like we
can add roles we won't be allowed to.

feat: implement a store for stat_traffic_data (#6190)
## About the changes

Implements a new store for collected traffic data usage that connects to
the new table `stat_traffic_data` primary key'd on [day, trafficGroup,
status_code_series].

Day being a date
Traffic group being which endpoint is being counted for, ie /api/admin,
/api/frontend etc
Status code series grouping 2xx status responses and 304 into their
respective 200 / 300 series.

No service here, this is for pro/enterprise

Feat/UI error observability (#6169)
This PR adds an endpoint to Unleash that accepts an error message and
option error stack and logs it as an error. This allows us to leverage
errors in logs observability to catch UI errors consistently.

Considered a test, but this endpoint only accepts and logs input, so I'm
not sure how useful it would be.

fix: version-service should not use process.nextTick (#6124)
In the beginning we used process.nextTick() as a trick to load some data
initally in the constructor of a service.

This is a bad pattern and we should generally avoid any async operations
in the constructor. Today we have two alternatives:

1. Defer loading until data is needed (wrap it in async)
2. Use the schdule-service.

fix: schduler-service graceful shutdown (#6125)
When a stop signal is sent to Unleash the scheduler-service should
cancel any scheduled jobs. This also applies to the job scheduled for
initial execution with jitter.


We observed that initial jobs was executed after the database
connections are terminated. This appears after v5.9.0 of Unleash.

chore: split metrics by request type (#6119)
## About the changes
Queries on client-feature-toggle store have many purposes depending on
the requestType, making the query more complex or not depending on the
use case. Also, each use case has different frequencies (i.e. playground
is expected to be used rarely).

The name for the store metrics was wrong, copy&pasted from:

7b04db0547/src/lib/features/feature-toggle/feature-toggle-store.ts (L107)

Which was also present in feature-tag metrics:

7b04db0547/src/lib/db/feature-tag-store.ts (L37)

With this, we'll have more granularity to understand the execution time
and frequency of each

fix: maintenance mode should assume disable if db call fails. (#6120)
Usually maintenance mode is disabled. If the call throws, which we see a
lot of when a unleash instance is in terminating state, we should return
a default value.

By having it throw inside of the memoizee function, the response is not
cached, and it will trigger new calls until it return a cachable result.

Fix/check maintenance flag once per minute (#6118)
## About the changes
Every schedule job will now check if maintenance is enabled. This ends
up querying the settings table in the db at least once per second per
running unleash instance. This small fix caches this query for 60
seconds to reduce the load somewhat.

We should reconsider this solution for the long term, but this will be a
great improvement on the short term.


**Logs after this fix running locally.** 
We can observe that we resolve settings from the DB once per minute.

Also we should consider giving a warning in section where you enable
maintenance mode that it can take up to a minute to propagate.

chore: scheduled created-by migrations metrics (#6089)
## About the changes

the created_by_user_id data migration from resolving events.created_by
(for both events and features) now emits events on how many rows were
updated.

Adds listeners for these events that records these metrics with
prometheus

fix: admin token should be passed forward from controllers (#5960)
We were sending `user.id` to the service, but if an admin token is used,
there is no `user.id.` Instead, there is
`user.internalAdminTokenUserId`. so we need to use the special method
`extractUserIdFromUser`.

This PR adds this implementation, and now the service correctly
retrieves the appropriate ID for admins.

chore: events created by userid migration (#6027)
## About the changes

Schedules a best-effort task setting the value of
events.created_by_user_id based on what is found in the created_by
column and if it's capable of resolving that to a userid/a system id.
The process is executed in the events-store, it takes a chunk of events
that haven't been processed yet, attempts to join users and api_tokens
tables on created_by = username/email, loops through and tries to figure
out an id to set. Then updates the record.

chore: implement created_by_user_id in features (#5994)
## About the changes

Adds a scheduled task that every 5 seconds updates 500 entries in the
features table setting `created_by_user_id`.
It does this by looking at the related event, checks created_by and
joins users table for match on username or email, and joins api_tokens
table on username matches. Then picks either a users id if set, or uses
-42 (admin token user)

fix: unauthorized disable feature (#5982)
## About the changes
This was spotted while testing automated actions. Steps to reproduce:

1. Add an editor user
2. Get a PAT for the editor user
3. As Admin create a feature in a project where the editor user is not a
member and enable the feature
4. Try using the editor's PAT to modify the feature
5. As the editor create a project (you'd be made owner) and try the same
request but just change the project name for the new project just
created (don't change anything else)

**Expected behavior**: you can't disable the feature
**Actual behavior**: the feature is disabled

This does not happen when trying to turn on a flag because during the
turn-on process we do validate if the feature belongs to project when we
call updateStrategy:

fix: feature toggle update total needs 4 labels (#5946)
So, this was causing a lot of ERROR in our logs, due to the metric
having gotten an extra label the last month.

Two things for this fix.
1. add the missing label to the two calls that did not have it added
2. update the log line to include the error as another argument to the
logger, so we actually get a stacktrace from the error.

Biome1.5.1 (#5867)
Lots of work here, mostly because I didn't want to turn off the
`noImplicitAnyLet` lint. This PR tries its best to type all the untyped
lets biome complained about (Don't ask me how many hours that took or
how many lints that was >200...), which in the future will force test
authors to actually type their global variables setup in `beforeAll`.

Adds a Biome rule for "no unused imports", which is something we
sometimes have trouble catching.

We're adding this as a warning for now. It is safely and easily fixable
with `yarn lint:fix`.

Two changes were needed to sort better

1. Since we are still using `last seen` from `features` table for
backwards compatibility, we needed to add it to sort condition.
2. Nulls break the order, so now sorting nulls as last.

 - Moves all "maintenance" files to a `maintenance` features folder
- Adds a `IMaintenanceStatus` read model that only includes

and expose only the `isMaintenanceMode()` method instead of the entire
`MaintenanceService`

chore: move enterprise check further left, prevent OSS from seeing CR usage (#5431)
This PR checks that the unleash instance is an enterprise instance
before fetching change request data. This is to prevent Change Request
usage from preventing OSS users from deleting segments (when they don't
have access to change requests).

This PR also does a little bit of refactoring (which we can remove if
you want)

chore: update segment cr return values (#5405)
This PR updates the returned value about segments to also include the CR
title and to be one list item per strategy per change request. This
means that if the same strategy is used multiple times in multiple
change requests, they each get their own line (as has been discussed
with Nicolae).

Because of this, this pr removes a collection step in the query and
fixes some test cases.

This PR handles the case where a single strategy is used in multiple
change requests. Instead of listing the strategy several times in the
output, we consolidate the entries and add a new `changeRequestIds`
property. This is a non-empty list that points to all the change
requests it is used in.

This is required for us to be able to link back to the change requests
from the UI overview.

feat: return CR uses of segments when flag is active (#5378)
This PR changes the payload of the strategiesBySegment endpoint when the
flag is active. In addition to returning just the strategies, the object
will also contain a new property, called `changeRequestStrategies`
containing the strategies that are used in change requests.

This PR does not update the schema. That can be done later when the
changes go into beta. This also allows us some time to iterate on the
payload without changing the public API.

## Discussion points:

Should `strategies` and `changeRequestStrategies` ever contain
duplicates? Take this scenario:
- Strategy S uses segment T.
- There is an open change request that updates the list of segments for
S to T and a new segment U.
- In this case, strategy S would show up both in `strategies` _and_ in
`changeRequestStrategies`.

We have two options: 
1. Filter the list of change request strategies, so that they don't
contain any duplicates (this is currently how it's implemented)
2. Ignore the duplicates and just send both lists as is.

We're doing option 2 for now.

functions.

This PR adds prometheus observability to our scheduled functions via a
summary. In addition to timing these functions with the more accurate
process.hrtime, we are getting a counter for free per scheduled job id.

fix: scheduler job runtime control (#5363)
## PR Description

Refactor with the goal of ensuring that flags are runtime controllable,
mostly focused on the current scheduler logic.

This includes the following changes:
 - Moves scheduler into its own "scheduler" feature folder
- Reverts dependency: SchedulerService takes in the MaintenanceService,
not the other way around
- Scheduler now evaluates maintenance mode at runtime instead of relying
only on its mode state (active / paused)
- Favors flag checks to happen inside the scheduled methods, instead of
controlling whether the method is scheduled at all (favor runtime over
startup)
 - Moves "account last seen update" to scheduler
 - Updates tests accordingly
 - Boyscouting

Here's a manual test showing this behavior, where my local instance was
controlled by a remote instance. Whenever I toggle `maintenanceMode`
through a flag remotely, my scheduled functions stop running:

Had a look through all of our current flags and it *seems to me* that
they are all used in a runtime controllable way, but would still feel
more comfortable if this was double checked, since it can be complex to
ensure this.

The only exception to this was `migrationLock`, which I believe is OK,
since the migration only happens at the start anyways.

## Discussion / Questions

~~Scheduler `mode` (active / paused) is currently not *really* being
used, along with its respective methods, except in tests. I think this
could be a potential footgun. Should we remove it in favor of only
controlling the scheduler state through maintenance mode?~~ Addressed in
7c52e3f638

~~The config property `disableScheduler` is still a startup
configuration, but perhaps that makes sense to leave as is?~~

Optimizations:

1. Removed extra round trip to database  to count environments
2. Removed extra round trip to database to count features

Fixes:

Currently, we were using a very optimistic query to set correct limit
and offset. This breaks as soon we we join tags.

            .limit(limit * environmentCount)
            .offset(offset * environmentCount);` 

The solution was to use common table expressions, so we could count and
rank features.

This PR hooks up the changes introduced in #5301 to the API and puts
them behind a feature flag. A new test has been added and the test setup
has been slightly tweaked to allow this test.

When the flag is enabled, the API will now not let you delete a segment
that's used in any active CRs.

fix: take into account project segments permission (#5304)

Fixes a bug where the `UPDATE_PROJECT_SEGMENT` permission is not
respected, both on the UI and on the API. The original intention was
stated
[here](https://github.com/Unleash/unleash/pull/3346#discussion_r1140434517).

This was easy to fix on the UI, since we were simply missing the extra
permission on the button permission checks.

Unfortunately the API can be tricky. Our auth middleware tries to grab
the `project` information from either the params or body object, but our
`DELETE` method does not contain this information. There is no body and
the endpoint looks like `/admin/segments/:id`, only including the
segment id.

This means that, in the rbac middleware when we check the permissions,
we need to figure out if we're in such a scenario and fetch the project
information from the DB, which feels a bit hacky, but it's something
we're seemingly already doing for features, so at least it's somewhat
consistent.

Ideally what we could do is leave this API alone and create a separate
one for project segments, with endpoints where we would have project as
a param, like so:
`http://localhost:4242/api/admin/projects/:projectId/segments/1`.

This PR opts to go with the quick and hacky solution for now since this
is an issue we want to fix quickly, but this is something that we should
be aware of. I'm also unsure if we want to create a new API for project
segments. If we decide that we want a different solution I don't mind
either adapting this PR or creating a follow up.

refactor: extract segment usage read model (#5301)
This PR adds a way to tell if a specific segment is being used in any
active change requests. It's the first step towards preventing segments
that are being used in change requests from being deleted.

It does that by checking the db for any unclosed CRs and using those CR
ids to look for "addStrategy" and "updateStrategy" events in the cr
events table.

## Upcoming PRs

This only puts in a way to detect it, but doesn't add that to anything.
That'll be in an upcoming iteration.

feat: replace gravatar-url with inline function (#5128)
As #4475 says, MD5 is not available in secure places anymore. This PR
swaps out gravatar-url with an inline function using crypto:sha256 which
is FIPS-140-2 compliant. Since we only used this method for generating
avatar URLs the extra customization wasn't needed and we could hard code
the URL parameters.

Refactor/separate client and admin store (#5006)
This PR is the first step in separating the client and admin stores.
Currently our feature toggle services uses the client store to serve
multiple purposes. 

Admin API uses the feature toggle service to serve both the feature
toggle list and playground features, while the client API uses the
feature toggle service to serve client features. The admin API can
change often and have very different requirements than the client API,
which changes infrequently and generally keeps the same stable structure
for long periods of time. This architecture is error prone, because when
you need to make changes to the admin API, you can very easily affect
the client API.

I aim to put up a stone wall between the two APIs. Complete separation
between the two APIs, at the cost of some duplication.

In this PR I have created a feature oriented architecture for client
features and disconnected the client API from the feature toggle
service. It now goes through it's own service to it's own store. For
feature toggle service I have duplicated and replaced the functionality
that serves /api/admin/features, I have kept a lot of the ugliness in
the code and haven't removed anything in order to avoid breaking
changes.

Next steps: 
* Move playground to admin API
* Remove client-feature-toggle-store from feature-toggle-service

feat: feature changes counted in new table (#4958)
As part of more telemetry on the usage of Unleash. 

This PR adds a new `stat_` prefixed table as well as a trigger on the
events table trigger on each insert to increment a counter per
environment per day.

The trigger will trigger on every insert into the events base, but will
filter and only increment the counter for events that actually have the
environment set. (there are events, like user-created, that does not
relate to a specific environment).

Bit wary on this, but since we truncate down to row per (day,
environment) combo, finding conflict and incrementing shouldn't take too
long here.

@ivarconr was it something like this you were considering?

This commit changes our linter/formatter to biome (https://biomejs.dev/)
Causing our prehook to run almost instantly, and our "yarn lint" task to
run in sub 100ms.

Some trade-offs:
* Biome isn't quite as well established as ESLint
* Are we ready to install a different vscode plugin (the biome plugin)
instead of the prettier plugin


The configuration set for biome also has a set of recommended rules,
this is turned on by default, in order to get to something that was
mergeable I have turned off a couple the rules we seemed to violate the
most, that we also explicitly told eslint to ignore.

This adds 2 methods to `EventService`:
 - `storeEvent`;
 - `storeEvents`;

This allows us to run event-specific logic inside these methods. In the
case of this PR, this means fetching the feature tags in case the event
contains a `featureName` and there are no tags specified in the event.

This prevents us from having to remember to fetch the tags in order to
store feature-related events except for very specific cases, like the
deletion of a feature - You can't fetch tags for a feature that no
longer exists, so in that case we need to pre-fetch the tags before
deleting the feature.

This also allows us to do any event-specific post-processing to the
event before reaching the DB layer.
In general I think it's also nicer that we reference the event service
instead of the event store directly.

There's a lot of changes and a lot of files touched, but most of it is
boilerplate to inject the `eventService` where needed instead of using
the `eventStore` directly.

Hopefully this will be a better approach than
https://github.com/Unleash/unleash/pull/4729

This PR updates the back-end handling of feature naming patterns to add
implicit leading `^`s and trailing `$`s to the regexes when comparing
them.

It also adds tests for the new behavior, both for new flag names and for
examples.

## Discussion points

Regarding stripping incoming ^ and $: We don't actually need to strip
incoming `^`s and `$`s: it appears that `^^^^^x$$$$$` is just as valid
as `^x$`. As such, we can leave that in. However, if we think it's
better to strip, we can do that too.

Second, I'm considering moving the flag naming validation into a
dedicated module to encapsulate everything a little better. Not sure if
this is the time or where it would live, but open to hearing
suggestions.

feat: make import/export work with project patterns (#4652)
This PR adds feature name pattern validation to the import validation
step. When errors occur, they are rendered with all the offending
features, the pattern to match, plus the pattern's description and
example if available.

To achieve this I've added an extra method to the feature toggle service
that checks feature names without throwing errors (because catching `n`
async errors in a loop became tricky and hard to grasp). This method is
also reused in the existing feature name validation method and handles
the feature enabled chcek.

In doing so, I've also added tests to check that the pattern is applied.

fix: EventStore#getMaxRevisionId can return null (#4384)
In a new fresh Unleash instance with cache enabled this can cause
feature toggles to never get updated.

We saw in our client that the ETag was ETag: "60e35fba:null" Which
looked incorrect for us.

I also did manual testing and if the andWhere had a value of largerThan
higher than whatever the id was then we would get back { max: null }.

This should fix that issue.

## Discussion points:

### getPlaygroundFeatures

The optional `includeStrategyId` parameter has been replaced by a
`getPlaygroundFeatures` in the service (and in the underlying store).
The playground was the only place that used this specific include, so
instead of adding more and making the interface for that method more
complex, I created a new method that deals specifically with the
playground.

The underlying store still uses an `optionalIncludes` parameter,
however. I have a plan to make that interface more fluid, but I'd like
to propose that in a follow-up PR.

feat: add "edit" link to playground strategies (#4027)
This change adds an "edit" link to all playground strategies when they
are returned from the API, allowing the user to jump directly to the
evaluated strategy edit screen.

This change applies to both old and new strategies, so it should even
work in the old playground.

This does not use this information in the frontend yet.

## Discussion points:

Should "links" be an object or a singular string? I know the
notifications service uses just "link", but using an object will make
it easier to potentially add more actions in the future (such as
"enable"/"disable", maybe?)

Do we need to supply basePath? I noticed that the notifications links
only ever use an empty string for base path, so it might not be
necessary and can potentially be left out.

## Changes

I've implemented the link building in a new view model file. Inspecting
the output after the result is fully ready requires some gnarly
introspection and mapping, but it's tested.

Further, I've done a little bit of work to stop the playground service
using the schema types directly as the schema types now contain extra
information.

This PR also updates the `urlFriendlyString` arbitrary to not produce
strings that contain only periods. This causes issues when parsing URLs
(and is also something we struggle with in the UI).

fix: reject API admin tokens when importing features (#4016)
This PR fixes an issue where trying to use an admin token to import
features via the API resulted in a 500 (due to missing properties).

The solution is to catch when the user is using and admin token in the
controller, throw a 400, and tell them to use personal access tokens or
service accounts.

The PR includes a new test file for this specific use case. We don't
really test these cases many other places, so it seemed the logical
choice.

feat: custom root roles (#3975)
## About the changes
Implements custom root roles, encompassing a lot of different areas of
the project, and slightly refactoring the current roles logic. It
includes quite a clean up.

This feature itself is behind a flag: `customRootRoles`

This feature covers root roles in:
 - Users;
 - Service Accounts;
 - Groups;

Apologies in advance. I may have gotten a bit carried away 🙈 

### Roles

We now have a new admin tab called "Roles" where we can see all root
roles and manage custom ones. We are not allowed to edit or remove
*predefined* roles.

This meant slightly pushing away the existing roles to `project-roles`
instead. One idea we want to explore in the future is to unify both
types of roles in the UI instead of having 2 separate tabs. This
includes modernizing project roles to fit more into our current design
and decisions.

Here I categorized permissions so it's easier to visualize and manage
from a UX perspective.

I'm using the same endpoint as before. I tried to unify the logic and
get rid of the `projectRole` specific hooks. What distinguishes custom
root roles from custom project roles is the extra `root-custom` type we
see on the payload. By default we assume `custom` (custom project role)
instead, which should help in terms of backwards compatibility.

### Delete role

When we delete a custom role we try to help the end user make an
informed decision by listing all the entities which currently use this
custom root role:

## Discussion points
- Are we being defensive enough with the use of the flag? Should we
cover more?
 - Are we breaking backwards compatibility in any way?
 - What should we do when removing a role? Block or demote?
- Maybe some existing permission-related issues will surface with this
change: Are we being specific enough with our permissions? A lot of
places are simply checking for `ADMIN`;
- We may want to get rid of the API roles coupling we have with the
users and SAs and instead use the new hooks (e.g. `useRoles`)
explicitly;
 - We should update the docs;
- Maybe we could allow the user to add a custom role directly from the
role selector component;

feat: improve frontend config freshness to < 1s (#3749)
This PR reuses the revision Id information from the "optimal 304 for
server SDKs" to improve the freshness of the frontend API config data.

In addition it allows us to reduce the polling (and eventually remove it
when we are confident).

Clean up old errors (#3633)
This PR attempts to improve the error handling introduced in #3607.

## About the changes

## **tl;dr:**
- Make `UnleashError` constructor protected
- Make all custom errors inherit from `UnleashError`.
- Add tests to ensure that all special error cases include their
relevant data
- Remove `PasswordMismatchError` and `BadRequestError`. These don't
exist.
- Add a few new error types: `ContentTypeError`, `NotImplementedError`,
`UnauthorizedError`
- Remove the `...rest` parameter from error constructor
- Add an unexported `GenericUnleashError` class
- Move OpenAPI conversion function to `BadDataError` clas
- Remove explicit `Error.captureStackTrace`. This is done automatically.
- Extract `getPropFromString` function and add tests

### **In a more verbose fashion**

The main thing is that all our internal errors now inherit
from`UnleashError`. This allows us to simplify the `UnleashError`
constructor and error handling in general while still giving us the
extra benefits we added to that class. However, it _does_ also mean that
I've had to update **all** existing error classes.

The constructor for `UnleashError` is now protected and all places that
called that constructor directly have been updated. Because the base
error isn't available anymore, I've added three new errors to cover use
cases that we didn't already have covered: `NotImplementedError`,
`UnauthorizedError`, `ContentTypeError`. This is to stay consistent in
how we report errors to the user.

There is also an internal class, `GenericUnleashError` that inherits
from the base error. This class is only used in conversions for cases
where we don't know what the error is. It is not exported.

In making all the errors inherit, I've also removed the `...rest`
parameter from the `UnleashError` constructor. We don't need this
anymore.

Following on from the fixes with missing properties in #3638, I have
added tests for all errors that contain extra data.

Some of the error names that were originally used when creating the list
don't exist in the backend. `BadRequestError` and
`PasswordMismatchError` have been removed.

The `BadDataError` class now contains the conversion code for OpenAPI
validation errors. In doing so, I extracted and tested the
`getPropFromString` function.

### Main files

Due to the nature of the changes, there's a lot of files to look at. So
to make it easier to know where to turn your attention:

The changes in `api-error.ts` contain the main changes: protected
constructor, removal of OpenAPI conversion (moved into `BadDataError`.

`api-error.test.ts` contains tests to make sure that errors work as
expected.

Aside from `get-prop-from-string.ts` and the tests, everything else is
just the required updates to go through with the changes.

## Discussion points

I've gone for inheritance of the Error type over composition. This is in
large part because throwing actual Error instances instead of just
objects is preferable (because they collect stack traces, for instance).
However, it's quite possible that we could solve the same thing in a
more elegant fashion using composition.

## For later / suggestions for further improvements

The `api-error` files still contain a lot of code. I think it might be
beneficial to break each Error into a separate folder that includes the
error, its tests, and its schema (if required). It would help decouple
it a bit.

We don't currently expose the schema anywhere, so it's not available in
the openapi spec. We should look at exposing it too.

Finally, it would be good to go through each individual error message
and update each one to be as helpful as possible.

### What
We've had this marked as deprecated through our v4, this PR removes it.

### Worth noting
This updates the deprecation notices with removal notices in the
documentation as well.

### Considerations
The tags API is still located under
/api/admin/features/{featureName}/tags. It should be moved to
/api/admin/projects/{project}/features/{featureName}/tags. I vote we do
that in a separate PR, we'd probably also need to deprecate the existing
tags endpoints for v5 and remove in v6. We could use 308s to signify
that they are moved.

feat: unify error responses (#3607)
This PR implements the first version of a suggested unification (and
documentation) of the errors that we return from the API today.

The goal is for this to be the first step towards the error type defined
in this internal [linear
task](https://linear.app/unleash/issue/1-629/define-the-error-type
'Define the new API error type').

## The state of things today

As things stand, we currently have no (or **very** little) documentation
of the errors that are returned from the API. We mention error codes,
but never what the errors may contain.

Second, there is no specified format for errors, so what they return is
arbitrary, and based on ... Who knows? As a result, we have multiple
different errors returned by the API depending on what operation you're
trying to do. What's more, with OpenAPI validation in the mix, it's
absolutely possible for you to get two completely different error
objects for operations to the same endpoint.

Third, the errors we do return are usually pretty vague and don't really
provide any real help to the user. "You don't have the right
permissions". Great. Well what permissions do I need? And how would I
know? "BadDataError". Sick. Why is it bad?

... You get it.

## What we want to achieve

The ultimate goal is for error messages to serve both humans and
machines. When the user provides bad data, we should tell them what
parts of the data are bad and what they can do to fix it. When they
don't have the right permissions, we should tell them what permissions
they need.

Additionally, it would be nice if we could provide an ID for each error
instance, so that you (or an admin) can look through the logs and locate
he incident.

## What's included in **this** PR?

This PR does not aim to implement everything above. It's not intended to
magically fix everything. Its goal is to implement the necessary
**breaking** changes, so that they can be included in v5. Changing error
messages is a slightly grayer area than changing APIs directly, but
changing the format is definitely something I'd consider breaking.

So this PR:

- defines a minimal version of the error type defined in the [API error
definition linear
task](https://linear.app/unleash/issue/1-629/define-the-error-type).
- aims to catch all errors we return today and wrap them in the error
type
-   updates tests to match the new expectations.

An important point: because we are cutting v5 very soon and because work
for this wasn't started until last week, the code here isn't necessarily
very polished. But it doesn't need to be. The internals can be as messy
as we want, as long as the API surface is stable.

That said, I'm very open to feedback about design and code completeness,
etc, but this has intentionally been done quickly.

Please also see my inline comments on the changes for more specific
details.

### Proposed follow-ups

As mentioned, this is the first step to implementing the error type. The
public API error type only exposes `id`, `name`, and `message`. This is
barely any more than most of the previous messages, but they are now all
using the same format. Any additional properties, such as `suggestion`,
`help`, `documentationLink` etc can be added as features without
breaking the current format. This is an intentional limitation of this
PR.

Regarding additional properties: there are some error responses that
must contain extra properties. Some of these are documented in the types
of the new error constructor, but not all. This includes `path` and
`type` properties on 401 errors, `details` on validation errors, and
more.

Also, because it was put together quickly, I don't yet know exactly how
we (as developers) would **prefer** to use these new error messages
within the code, so the internal API (the new type, name, etc), is just
a suggestion. This can evolve naturally over time if (based on feedback
and experience) without changing the public API.

## Returning multiple errors

Most of the time when we return errors today, we only return a single
error (even if many things are wrong). AJV, the OpenAPI integration we
use does have a setting that allows it to return all errors in a request
instead of a single one. I suggest we turn that on, but that we do it in
a separate PR (because it updates a number of other snapshots).

When returning errors that point to `details`, the objects in the
`details` now contain a new `description` property. This "deprecates"
the `message` property. Due to our general deprecation policy, this
should be kept around for another full major and can be removed in v6.
